Children’s Healthcare of Atlanta trauma doctors are treating three to four times more e-bike injuries than a year earlier, after a 14-year-old Johns Creek boy broke his femur when a car struck him on a Class 3 machine. National estimates show injuries to riders 14 and under jumped 167 percent in a single year as sales climbed and state rules left most age decisions to parents.
The local spike sits inside a national pattern that CPSC data and independent analysis of the same system both document: machines capable of 20 to 28 mph are sold and ridden like ordinary bicycles by kids who have never navigated traffic at those speeds.
Carter Moore’s Femur and the Call That Changed Everything
On May 5, Danielle Moore got the call every parent dreads. Her son Carter, 14, was riding his e-bike home from football practice on Nesbit Ferry Road in Johns Creek when a car hit him. He flew off the bike and flipped over the car.
“I was like, ‘Oh crap! I’m gonna hit this car! I hope I don’t die,’” Carter later said. He spent three days at Children’s Healthcare of Atlanta with a broken femur. More than three months later he still finds stairs painful and says he will never ride an e-bike again.
“It was just fear and just sorrow and dread, just seeing my child laying on the street and he’s reaching for me,” Moore said. She learned only afterward that sidewalk riding is illegal for older kids in many areas and that e-bikes come in distinct speed classes. Carter had been on a Class 3 model, pedal-assist up to 28 mph.
A related local report put the Children’s trauma count at two e-bike cases in 2024 and 24 in 2025, with 2026 still climbing. Dr. Sofia Chaudhary, who treats these patients, described the broader shift plainly.
I’d say we’re seeing three to four times the number of injuries for e-bikes… anywhere from fractures of the arms and legs to significant and severe facial injuries, head injuries, trauma from brain injuries.

National Numbers Climb Faster Than Most Realize
The Georgia cases track a steep national curve. CPSC staff estimated 59,200 e-bike emergency visits in 2024, up from an annualized average near 6,300 in the 2017-2021 period. That single year accounted for 38 percent of the 155,200 e-bike ED visits tallied from 2017 through 2024. Fatalities reached 310 over the same span, with motor-vehicle crashes and loss of control the leading hazards.
An independent analysis of NEISS data published in August 2026 by ERideHero put 2025 e-bike ER visits at roughly 102,777, a 71 percent rise from the prior year and a nineteen-fold jump since 2020. Children 14 or younger accounted for an estimated 17,206 of those visits, up 167 percent from 6,444 in 2024. Their share of all e-bike injuries rose from 10.7 percent to 16.7 percent in twelve months. Hospitalization or observation rates hit 17.6 percent, higher than for e-scooters or conventional bikes. Nearly three in ten injuries involved the head or face, yet only about 40 percent of injured riders in the CPSC’s 2024 follow-up wore helmets.

Males made up about 78 percent of injured riders. Summer months dominate; July 2025 alone produced an estimated 13,140 injuries. Twenty-four percent of the 2024 special-study cases involved a motor vehicle, 58 percent occurred on paved roads, and 15 percent of riders were traveling 20 mph or faster at impact.
Three Classes, Almost No Age Floor for Most Bikes
Georgia follows the common three-class system. Class 1 is pedal-assist only, motor cuts at 20 mph. Class 2 adds a throttle to the same 20 mph ceiling. Class 3 is pedal-assist to 28 mph and usually requires a speedometer. State law sets a clear floor only for the fastest category.
Under no person under 15 shall operate a Class III electric assisted bicycle, though a younger child may ride as a passenger on a model designed for it. Every Class 3 operator and passenger must wear a bicycle helmet. Class 1 and Class 2 carry no statewide minimum operator age. Riders under 16 still fall under the general bicycle helmet rule on roads, paths, and lanes, but nothing stops a younger child from legally operating a throttle-equipped 20 mph machine.

Local rules tighten further. The Atlanta Beltline limits riders to Class 1 and 2 and a 15 mph speed cap. Sidewalks are off-limits for bikes in many jurisdictions once a child is past early elementary age, a fact Moore said her family did not know until after the crash. Earl Serafica, who sells e-bikes at Specialized on the Beltline, told Atlanta News First he would keep kids off any e-bike until 16 or 17 so they first understand traffic.
Where the Crashes Happen and Who Gets Hurt Worst
CPSC’s special study of 2024 e-bike cases found most injuries on paved roads rather than paths. Visibility problems contributed in about 10 percent of incidents; only 44 percent of those riders had lights or reflective gear. Carrying bags or other items was common. Distraction from phones or music appeared in just 3 percent of reports, suggesting speed and road-sharing, not multitasking, drive most of the harm.
Speed-related trauma shows up clearly in pediatric data. One national analysis of pediatric e-bike patients found speed-attributed injuries linked to higher rates of head, neck, or facial trauma and internal organ injuries, nearly all of them to the head and neck. Fractures lead diagnoses. Older adults account for a disproportionate share of deaths even though younger riders fill more ER beds; fragile bones and slower recovery reverse the age pattern once severity rises.
On X and in clinics the same observation keeps surfacing: parents buy what looks like a bicycle and discover only after a crash that the motor and weight turn ordinary mistakes into high-energy events. One widely viewed post from a healthcare worker urged parents simply not to buy an e-bike for a child, citing life-changing and deadly outcomes. Another Atlanta-area observer described two kids sharing an e-bike at what looked like 40 mph with no helmets, weaving through traffic. The product classification gap turns suburban after-school rides into trauma activations.
Helmets, Training, and the Parent as Last Line
Chaudhary put the responsibility where the statute leaves it. “You set the safety rules and you set the safety standards,” she said. Serafica recommends a MIPS helmet that can shift on multi-directional impact rather than a basic bike lid. AAA has warned that ordinary bicycle helmets are rated only to about 28 mph; faster speeds or higher-energy crashes push the need toward motorcycle-grade protection, though few kids wear even a basic helmet consistently.
The American Academy of Pediatrics-aligned guidance on HealthyChildren.org is more restrictive than Georgia law: children age 9 through 12 should not operate any product faster than 10 mph, meaning no e-bikes under 12, and Class 1 only for carefully vetted 12-to-15-year-olds after training. Propel ATL and Specialized have offered free e-bike safety classes with Specialized covering battery handling, road sharing, and basic maintenance, though those sessions have been limited to adults 18 and older.
Practical steps that keep recurring in doctor and retailer interviews include:
- Delay any e-bike until the rider can handle traffic and has mastered a conventional bike
- Match the class to the child and never start with Class 3
- Require a properly fitted, strapped helmet every ride, preferably MIPS or higher-rated
- Teach that sidewalks are usually illegal and that cars often fail to see or yield to quiet e-bikes
- Check local path and Beltline rules before every ride
- Consider a formal safety class even if the shop’s version is adult-only
What the Mismatch Leaves Behind
Carter Moore still feels the broken femur every time he climbs stairs. His mother now knows the sidewalk rule and the class system that she had never heard of before the crash. Doctors at Children’s keep seeing the same pattern: higher speeds, heavier machines, younger riders, and injuries that look more like motorcycle trauma than playground falls.
Georgia’s statute draws a bright line only at Class 3 and age 15. Everything slower remains a parental judgment call while national child injury counts keep multiplying. The second-order effect is already visible in Atlanta trauma bays and in the 17,000-plus kids under 15 who reached ERs in 2025. Until age floors, helmet mandates, or product labeling catch up to the motors, the hospitals and the families will keep absorbing the difference between a bicycle and a 28 mph vehicle.
